چکیده

Ahmet Çolak, Dr., MD. Department of Neurosurgery, GATA Haydarpaþa Research and Training Hospital, [email protected] Abstract Isolated oculomotor palsy due to vascular compression at the root exit zone is an extremely rare entity. A 59-year-old male patient was presented with complaints of diplopia and mild ptosis. A thin-sliced MRI demonstrated that, the right posterior cerebral artery was impinging on the right oculomotor nerve at the root exit zone. Carbamazepine therapy was initiated, and after a month of medication, both diplopia and ptosis had completely resolved. Although it is extremely rare, vascular compression of the oculomotor nerve at the root exit zone should also be considered in the differential diagnosis of isolated oculomotor palsy.

Complete Oculomotor Nerve Palsy Caused by Direct Compression of the Posterior Cerebral Artery.

Oculomotor nerve palsy frequently occurs because of external compression by an internal carotid-posterior communicating artery aneurysm and diabetes mellitus. In addition, pontine infarction, cavernous sinus tumors, demyelinating disease, and autoimmune disorder are well-known causes of oculomotor nerve palsy. However, cases of complete oculomotor nerve palsy by neurovascular conflicts presente...

Ptosis as Partial Oculomotor Nerve Palsy Due to Compression by Infundibular Dilatation of Posterior Communicating Artery, Visualized with Three-Dimensional Computer Graphics: Case Report

Oculomotor nerve palsy (ONP) due to internal carotid-posterior communicating artery (PcomA) aneurysm generally manifests as partial nerve palsy including pupillary dysfunction. In contrast, infundibular dilatation (ID) of the PcomA has no pathogenic significance, and mechanical compression of the cranial nerve is extremely rare. We describe a 60-year-old woman who presented with progressive pto...

Transient Bilateral Oculomotor Nerve Palsy (TOP) Associated with Ruptured Anterior Communicating Artery Aneurysm: A Case Report

Unilateral oculomotor nerve palsy has been recognized as a localizing sign due to aneurysms along the internal carotid artery, particularly at the internal carotid-posterior communicating artery (ICA-PcomA) junction and is considered to be a variable symptom caused by direct pressure on the nerve. Therefore bilateral pupillary dilation in ruptured cerebral aneurysm cases can be misleading in in...

Unilateral Oculomotor Nerve Palsy as an Initial Presentation of Bilateral Chronic Subdural Hematoma: Case Report

Isolated oculomotor nerve palsy is well known as a symptom of microvascular infarction and intracranial aneurysm, but unilateral oculomotor nerve palsy as an initial manifestation of chronic subdural hematoma (CSDH) is a rare clinical condition. We report a rare case of an 84-year-old woman with bilateral CSDH who presented with unilateral oculomotor nerve palsy as the initial symptom. The pati...
